The process of dying helps form our fears of death. What we see other people going through while they are dying prompts us to ask ourselves questions. Will there be a lot of pain? Will I be alone? What about my physical appearance?
Fear of the unknown affects our fears about death. Religious beliefs or spirituality may affect our beliefs. People may ask themselves about after life. Is there punishment? Is there heaven?
Many people fear death because they are afraid of what they will be leaving behind. They are afraid that their family won't be able to cope with the stress of their death, the financial burden, and if someone is to take their place in the family life.
Another reason to fear death is when the person does not feel like they have accomplished everything they were put on this earth to do. In essence, their lives "in box" will still have things to be done left in it when they pass on.
An additional point is that many people do not fear the not being alive, they fear what happens during the 'dying' process. They more fear the fact that many 'killers', like cancer and terminal illnesses like hepatitis have painful side effects.
